ANCHORAGE, Alaska (AP) - A preliminary report says a pilot saw a flash on his left side just before his plane collided with another plane in Alaska, killing six people. The National Transportation Safety Board says in the report released Wednesday that the two planes carrying Alaska cruise ship passengers were returning from a flightseeing tour of Misty Fjords when the collision occurred May 13 northeast of Ketchikan.

On May 21, 2019 at 2136 hours Alaska State Troopers responded to a report of a motor vehicle collision on Chena Pump Road. Troopers contacted the driver, Leonard Contreras Guerrero, age 30 of Fairbanks. Investigation resulted in the arrest of Contreras Guerrero for Driving Under the Influence. He was remanded to the Fairbanks Correctional Center for Driving Under the Influence and Refusal to Submit to a Chemical Test.

On May 21, 2019 at 1823 hours Alaska State Troopers contacted a disabled vehicle on Chena Ridge Road. Troopers identified Chad Garrett, age 48 of Fairbanks, who provided a false name. Investigation resulted in the arrest of Garrett, who was remanded to the Fairbanks Correctional Center on charges of False Report and Driving While License Revoked.

On 5/21/2019 at 2331 hours, AST dispatch received a call reporting a single vehicle crash at Old Steese Highway near Goldstream Road. The caller reported a motorcyclist, later identified as 38-year-old Alexander Holder of Fairbanks, was traveling at a high rate of speed, lost control, and went into the trees; Holder was ejected into a creek. The Steese Highway Fire Department arrive on scene and started life saving measures. Life saving measures failed and Holder was pronounced deceased on scene on 5/22/2019 at 0001 hours. The State Medical Examiner’s Office was notified and requested Holder’s body be sent to Anchorage for autopsy. Next of kin has been notified. Holder was wearing a helmet at the time of the collision.

On 05/19/19, at 1725 hours, Alaska State Troopers responded to Three Bears in Meadow Lakes for the report a shoplifter in custody. AST contacted Korby Moorman, 25 yoa of Wasilla. Investigation revealed Moorman took a knife set valued at under $50 and concealed it in his clothing. A check of APSIN revealed Moorman had three outstanding Arrest Warrants for Failure to Appear totaling $400. Mooman was arrested without incident and transported to Mat-Su Pretrial where he was remanded.

On 05/19/2019 at approximately 0824 hours, AST received a report of a burglary off the Parks Hwy just south of Nancy Lake. Investigation revealed someone entered the property sometime between 5/15/2019 at 0700 hours and 5/16/2019 at 0730 hours while the victim was out of the residence. The suspect attempted to enter a building on the property by smashing in the door and was unsuccessful. The residence sustained damage to the garage door and the rear door of the house. The damage was estimated at approximately $400.00. The estimated value of stolen property is approximately $13,307.00. Investigation continues.

Brown was arrested and transported to Mat Su Pretrial where he was remanded on the charge of Assault 4th Degree DV.

Investigation revealed that Julia Sunnyboy, age 42 of Talkeetna, had an outstanding warrant for her arrest. Sunnyboy was arrested and transported to the MATSU Pre-Trial facility where she was remanded with a bail of $2,500 cash appearance.

On 5-20-19 at 1214 hours, Trooper dispatch received a request for assistance for a possible civil type issue at an address on Pioneer Peak Dr in Wasilla. After troopers responded, it was discovered the glass from the door was busted out at the residence and a dead bolt lock had been drilled out. Investigation shows Matthew Smith, age 32 of Wasilla, is the person that damaged the property. He was arrested for Criminal Mischief 4th Degree DV and transported to Mat Su Pretrial where he was held pending arraignment.

On 05/21/19, at approximately 1335 hrs. Alaska State Troopers received a request to conduct a welfare check on an elderly female that wasn’t answering her phone for three days. A family friend was at the home and made entry and located 72 year old, Toni Everman deceased in the back room. The State Medical Examiner and Alaska State Troopers responded to the residence off Twin Peaks in Palmer. The scene investigation and the State Medical Examiner concluded foul play was not a factor and Everman died of natural causes.

On 5/19/19, at 1455 hours, AST responded to a residence on Red Fox Drive in Wasilla for a reported assault between neighbors. Investigation revealed that Roland H. Kroener, 55 yoa of Wasilla, placed a neighbor in fear of imminent serious physical injury by charging at the neighbor while wielding a splitting maul and threatening to kill the neighbor. The Palmer DA's office was consulted and requested charges of Assault 3 be forwarded for review.

On 5-21-19 at 1032 hours, Trooper dispatch received a report of a theft from a building on a property on Cheri Lake Dr. Investigation reveals someone entered the unlocked building and took multiple items sometime between 5-2-19 and 5-10-19. On 5-19-19 at 1131 hours, Trooper dispatch received a report of a mail theft from a porch on Fernwood Dr in Wasilla. Investigation shows at least two mail packages left on the porch were taken sometime on 5-17-19 between 1300 and 1730 hours. Palmer Police contacted a suspect with a package and other items belonging to the victim. Those items were recovered. The investigation is ongoing at this time.

On 5-20-19 at 0800 hours, Trooper dispatch received a report of a theft from a vehicle parked at a residence on Forest Grove Circle. Investigation reveals someone took a pair of custom roller skates from the unlocked vehicle sometime the night before. The skates are valued around $500 and were described as a Crazy brand skate. On 5/20/19, at 0103 hours, AST was contacted by Lawrence Shepard who reported running aground in a slough off the Susitna River just downriver from the Deshka Landing; but well out of sight of normal river traffic. Shepard reported he was unable to free the vessel or get to shore. At approximately 0945 hours, AWT Troopers responded to the location in a state jetboat and transported Shepard back to the Deshka Landing. Shepard was uninjured but cold.

The complainant reported person(s) unknown had cut chains and locks and accessed a connex and RV stealing a set of 42 inch mud terraign "swamp" tires, and various tools. On 5-20-19 at 1102 hours, Trooper dispatch received a report that someone took a mini-bike from a residence on Williwaw Way. Investigation shows someone took a small motor bike that was described as red or maroon with an open tubular frame and a lawn mower engine. The bike is valued around $75 and was taken within the previous two days. Troopers received a lead on the bike later the same day. The investigation is ongoing.

Investigation shows someone stole a purple and gray 1994 Chevrolet Camaro from a yard at the residence. The vehicle was inoperable at the time and was towed off the property sometime between 2200 hours the night before and 1300 hours on 5-21-19. On 5-20-19 at 1005 hours, Trooper dispatch received a report of a burglary that occurred at a residence on Chickaloon Road in Wasilla. Troopers responded to investigate. Investigation reveals someone broke into the residence within the previous two weeks and stole three rifles and two pistols. The pistols were described as a Ruger .22 cal revolver and a Ruger Black Hawk stainless .44 Magnum. The rifles were described as a Winchester model 70 .270 cal, a Remington Model 7600 pump action 30.06 and a Thompson Center Arms model Triumph .50 cal muzzle loader. On 5/19/19, at 2117 hours, AST responded to a reported ATV rollover with minor injuries on Wasilla Fishhook Road in Wasilla. Investigation revealed the operator, Kristopher T. Adams, 24 yoa of Wasilla, was operating the ATV while under the influence of alcohol. Adams was arrested for DUI and endangering the welfare of a minor, transported to Mat-Su pretrial and held on $500 bail.

On 5/21/2019 at approximately 2324 hours, Alaska State Troopers stopped a red 2000 Chevrolet Tahoe on Mackey Lake Road for a traffic violation. Tyler Joseph Fuxa, age 27 of Soldotna, was located under a blanket in the vehicle. Investigation revealed he had two outstanding arrest warrants for Domestic Violence assaults. He was arrested and remanded at Wildwood Pretrial Facility where he was held with no bail pending arraignment.

Investigation revealed Scott Toloff, age 56 of Nikiski had violated the protective order against him. AST was unable to locate Scott and a warrant for his arrest has been requested.

On 5/21/19 at approximately 1304 hours the Alaska State Troopers received report of a theft at Hope Community Resources. Staff at Hope reported that over the weekend someone had broken the latch on the shed but did not take anything, then over Monday night someone had returned and taken 2 propane bottles from outside the building as well as a table and two chairs. If anyone has information regarding this case they are encouraged to call AST at 907-262-4453. Investigation is ongoing.

On 5/19/19 Alaska Wildlife Troopers Sitka Post contacted Michael Jacoby, age 40, of Sitka harvesting abalone in Eastern Channel near Sitka. Investigation revealed Jacoby was in possession of three abalone less than the 3.5 inch minimum size. Jacoby was cited with bail set at $150 in the District Court at Sitka.

On 5/21/19 Alaska Wildlife Troopers Sitka Post cited Gary Cranford, age 43 of Sitka, for operating personal use shellfish pots without a biodegradable escape mechanism. Bail is set at $120 in the District Court at Sitka.

On 5/21/19 at approximately 1513 hours, Alaska State Troopers and Alaska Wildlife Troopers were notified of an aircraft crash in Cascade Bay within Prince William Sound. The USCG and RCC responded to the scene. Investigation revealed that Scott Johannes age 56, of Wasilla, was flying his aircraft with two passengers on board. Johannes was attempting to land in Cascade Bay, but ended up crashing into the water. Johannes an one passenger were able to escape the aircraft. William Resinger, 75 of Palmer, was trapped inside the overturned aircraft. Several good Samaritans were in the area and responded in boats. Resinger was extricated and CPR was initiated but unsuccessful. Johannes and the surviving passenger were transported to an Anchorage hospital. The SME was notified and responded. Palmer Next of kin have been notified for Resinger. The NTSB and FAA have been notified. The investigation continues.
